locations. The plot plan must be attached to the online application.
* Applications signed by any person other than the legal owner of the property must complete the
Designation of Legal Representation form and must attach the form to the application.
*** All Attachments will be requested with the application, after the electronic signature.

- DELIVER TO THE OFFICE AT: 1048 CARRIAGE OAKS DRIVE, CARTHAGE.
* ALL FEES ARE NON-REFUNDABLE AFTER NINETY (90) DAYS OR AFTER VISIT TO THE
PROPERTY HAS BEEN MADE.
* APPLICATIONS ARE PROCESSED ON A FIRST COME/FIRST SERVED BASIS. IF YOUR TURN
COMES AND THE ABOVE HAS NOT BEEN COMPLETED, THE NEXT APPLICATION LINE WILL BE
PROCESSED.
* APPLICANT IS REQUIRED TO UNCOVER THE SEPTIC TANK PRIOR TO THE SITE VISIT.
* APPROVAL EXPIRES SIX (6) MONTHS FROM DATE OF ISSUANCE.
