HOW TO SUBMIT YOUR PLAYER’S PROOF OF AGE IF NOT A RETURNING REGISTRANT:
A first time registrant must submit proof of age. This can be in the form of a birth certificate, State or school ID Card, Military ID , School Shot record, or Medicaid Card. What is required is that the players name and date of birth both appear on this identification.
You can submit your player’s proof of age in several ways. You can attach before submitting this form, scan and email it to tkirk@moorecountync.gov, FAX a copy to 910 947-1627, mail a copy to PO 905 Carthage, NC 28327 or stop by the Moore County Parks and Recreation office at 302 Monroe Street, Carthage NC 28327 and show it to a staff member. Special Requests or Assignments can only be made by calling the Parks and Recreation directly at 910.947.2504.
